Noise and background speech: in noisy spaces (e.g. an exam room) distant conversation may appear, incorrectly, as medical text in the transcript.
Recognition errors: pronunciation, technical terms, mixed languages or a poor microphone can reduce accuracy.
Hallucination: automatic summarization can occasionally generate content that was not spoken.
Language coverage: quality may vary by language.
Risk mitigation: mandatory professional review of every report, use of a suitable microphone, and noise reduction. The vast majority of errors stem from inadequate audio capture (since the system then tries to summarise from poor source data); if you experience errors, it is always worth starting with the audio capture setup.
